The Parent Consent Form (PCF) must be completed annually for all students.
All care for injuries or illness is handled in the school clinic. In case of a serious injury or illness, a member of the school staff will call 911 and contact a parent. It is, therefore, vital to have current emergency information on file. Be sure and keep the office updated regarding any changes to contact numbers. All students checking out of school due to illness must first be seen in the school clinic.
